In June 2026 we ran the questions Chicago patients actually type on Perplexity, one of the major AI answer engines. Three patterns held — and every one is something a practice can act on.

Finding 1

AI ranks dermatologists by your reviews.

Ask “best dermatologist in Chicago” and AI returns a shortlist justified by Google rating and review count — “a 4.8 with 1,400+ reviews” — pulled from the map results box at the top of Google and health directories like Healthgrades, Zocdoc, and Yelp. Finding 2

Every condition is a separate race.

We changed “best dermatologist” to “best dermatologist for acne” and AI returned a different set of practices — the ones that had published a dedicated acne page, alongside condition-specific directory listings. For each condition you treat — acne, eczema, skin cancer, or cosmetic — there’s a separate answer to win, and a generic homepage wins none of them.

Finding 3

It’s all third-party — not your homepage.

The sources behind the answers were Healthgrades, Zocdoc, Yelp, and MediFind, plus practices’ Google profiles and individual condition pages — not a practice’s homepage. Board certification and a respected affiliation help, but a practice with a thin or inconsistent web presence simply doesn’t appear.

A small, transparent sample, not a full index; AI answers vary by person and location and change over time. Specific practices are intentionally not named here.
