Why isn’t my med spa showing up in AI search?
Usually because nobody except you is saying you’re good — at least not anywhere the AI reads. Ask ChatGPT, Gemini or Perplexity why your clinic is invisible and you’ll get the same checklist every time: complete your Google Business Profile, add structured data, write a page per treatment. That advice is real, and you should do it. But it only makes you readable. It doesn’t make you recommended.
The two are separate, and the gap is measurable. In a June 2026 controlled study, a brand’s own page was cited as a source while the AI went on to recommend its competitors about 69% of the time.1 The assistant read the site, used it, and still sent the customer elsewhere.
What we measured
We ran the questions patients actually ask — best clinic nearby, is it worth it, safest place to go — across every major engine for six clinics, and recorded 216 question–answer pairs. The typical clinic was named in about a third of answers. One clinic with 587 reviews was named in just 14%.2
That last number is the one worth sitting with. Five hundred and eighty-seven happy patients is a strong reputation by any normal measure, and it bought almost nothing in the answer. Reviews on their own don’t carry it, because the assistant isn’t counting your reviews — it’s looking for agreement across the sources it already trusts.
Three checks you can run in ten minutes
- Ask, don’t search. Type the question a patient would type — “best med spa for Botox in [your town]” — into ChatGPT, Perplexity and Google’s AI Mode. Write down who gets named. Ask each engine separately: they draw on different sources and routinely disagree.
- Read the sources, not the answer. Click the links the AI cites. Those pages — directories, roundups, review sites, local press — are the ones deciding the answer. Are you on them? Accurately?
- Ask the engine why. Follow up with “why didn’t you mention [your clinic]?” It will often tell you plainly. When we asked it about ourselves, it said we were missing from industry directories and had almost no footprint it could verify.3
What actually moves it
AI recommends the clinic that sources outside your control agree on.
That means accurate listings wherever patients and engines look, genuine reviews that describe specific treatments, and real mentions in places that write about clinics in your area. It’s slower than buying an ad, and it’s the part you keep: an ad stops the day you stop paying, and an earned mention keeps being read.
One more thing our own runs show: engines differ sharply in how hard they are to win. In our August audit, four engines named us for a question and Google’s AI Overviews still didn’t — the strictest surface of the five.4 Being recommended by one assistant is not being recommended by all of them.
We measure this on ourselves too
Every month we run the same audit on Oracite and publish it, including the bad months.
